It is composed of nine villages: Blidari, Boiștea, Căiuți, Florești, Heltiu, Mărcești, Popeni, Pralea, and Vrânceni.

The commune is situated on the Moldavian Plateau, at an altitude of Template:Cvt, on the banks of the Trotuș River and its right tributary, the Căiuți. It is located in the southern part of Bacău County, Template:Cvt south of the county seat, Bacău, on the border with Vrancea County. Căiuți is crossed by national road Template:Ill, which connects Onești to Bârlad.
