Dear candidate, thank you for offering to serve Wikipedia as a CheckUser. You may wish to answer the following optional questions to provide guidance for participants:

1. Have you read and understood the English Wikipedia CheckUser policy and the Foundation's CheckUser policy? Do you understand the role of a CheckUser?

2. Do you meet the Foundation's minimum requirements for CheckUsers (which include being at least 18 years of age)? Are you willing to submit verification of your identity to the Foundation in order to prove this?

3. Do you pledge that you will respect the privacy of editors, and that you will not use the CheckUser tool except in a manner permitted by policy?

4. If you are already serving as an administrator or bureaucrat, have you been involved in any conflicts involving your use of the tools?
