Summary

Based on the standardized Kt/V as calculated by an equation proposed by Leypoldt (see text for reference). To use, take the single-pool Kt/V on the horizontal axis, move up to the isopleth (curve) corresponding to the number of sessions per week, and read the weekly standard(ized) Kt/V on the vertical axis. An stdKt/V value of 2.0 is considered to be a minimum amount for patients with no residual renal function, and corresponds to a spKt/V of 1.2 with a 3x/week dialysis schedule. The equations are very sensitive to session length (t) as well. To make this nomogram, it was assumed that dialysis was being delivered at a rate of 0.4 spKt/V units per hour, but session length was not allowed to go lower than 120 min (That's why, if you look carefully, you will see an inflection in the isopleths at an spKt/V of 0.8).
